Hi fellow coders,
I am having taking the CHONC again. I failed by 1 point with a 69☹️. Is the anyone that has the CHONC that can tutor me or point me in the direction that I can get some help. Lauren or any instructor do you know if you will have a webinar regarding the CHONC exam help? I am so close please someone help me!!!!!!!
 

The CHONC certification exam will test:​

  • Ability to read and abstract physician office notes and operative notes to apply correct ICD-10-CM, CPT®, HCPCS Level II, and modifier coding assignments
  • Rules and regulations for Medicare billing — including incident to, teaching situations, shared visits, consultations, and global surgery
  • Coding of surgical procedures performed by hematologists and oncologists and auxiliary staff such as chemotherapy administration, hydration services, therapeutic, prophylactic and diagnostic administration, bone marrow aspiration, bone marrow biopsies, venipuncture, vaccinations, therapeutic phlebotomy, etc.
  • 2021 evaluation and management (E/M) guidelines for new and established office and outpatient services, as well as E/M coding based on 1995 and 1997 documentation guidelines
  • Medical terminology
  • Anatomy and physiology

CCO does not have a CHONC course although we were considering it at one time.

Shared visits

For 2022, such visits may be billed under the National Provider Identifier (NPI) of the physician or non-physician practitioner (NPP) who either (1) documents the support for the history, exam, or medical decision-making for the visit, or (2) provides more than 50% of the service time. For 2023, split/shared visits must be billed under the NPI of the individual who provides more than 50% of total visit time.

These new rules could significantly impact Medicare reimbursement for physician practices that use NPPs in facility settings. For evaluation and management (E/M) visits jointly furnished by a physician and NPP in the same group practice in a facility setting (e.g., place of service [POS] 21 inpatient hospital or 22 provider-based clinic), CMS has permitted the visit to be billed under the physician’s NPI. Specifically, CMS’ split/shared visit rule has required that a physician perform only some portion of the E/M visit in order to bill that service under the physician’s NPI. Thus, the practice received full payment for the visit rather than the 85% of the Physician Fee Schedule rate paid for services billed under an NPP’s NPI.

In the Final Rule, CMS clarified and made several changes to its rules for split/shared visits:

  • The split/shared visit rules do not apply to office visits (place of service 11); instead, these visits may be billed ‘incident to” if the requirements are satisfied (established patient, established plan of care/condition, direct supervision). CMS has signaled that it will be reviewing the incident to rules in future rulemaking.
  • CMS now will permit split/shared visits to be reported for new patients as well as established patients, for initial as well as subsequent visits, for critical care services, for prolonged E/M visits, and for skilled nursing facility/nursing facility E/M visits (other than those required to be performed in their entirety by a physician).
  • CMS will require the use of a new modifier (yet to be identified) to identify all claims for split/shared visits.
  • Documentation in the medical record must identify the two individuals (physician and NPP) who performed the visit.
  • A split/shared visit must be billed under the NPI of the individual who performed the substantive portion of the visit. That individual also must sign and date the medical record.
    • For 2022 only, split/shared visits may be billed under the physician’s NPI if (1) the medical record documentation indicates the physician performed one of the three key components (history, exam, or medical decision-making) in its entirety, or (2) more than half of the total time for the visit was spent by the physician. Otherwise, the visit must be billed under the NPP’s NPI. (For critical care services, only time may be used.)
    • Beginning in 2023, only time will be used to determine the substantive portion of the visit.

I just spend a week auditing our Hemo/Onco providers at BSW. Some key features I would way that needed improvement which makes me think it would be tested on is Primary and Secondary CA.
Was there an area you felt you needed help in?
 
Thank you all for your advice and the materials. I will use them. These are the areas I need work. Will these resources help my e/m theory. I think I am falling short there. Fun fact I taught my self all this information so I am proud I received a 69 for self taught. Lol any help will be big to me.

"Free is awesome! If you're interested in pursuing free YouTube lectures and self-studying during your journey, then we're all for it. However, if there's one thing we've learned after releasing 700+ free videos on YouTube over the last 10 years, it's that learning "for free" requires a lot of time, research and support along the way. And most free videos simply don't help students that are watching for free which causes quite a bit of frustration along the way. So if you think you might need a little help understanding any concepts or strategies, you should consider getting certified staff support.

Many people like you have had some sort of training - perhaps a few years ago - or have experience and are wondering if they really need a full Course or if a Review Blitz and some studying would be enough to pass the AAPC exam.

We recommend that you take a free Practice Exam for your desired credential in two hours and see how you score to decide.

If you score low - like below 65 or less - you may want to consider taking a full Course to relearn the material.

If you score 65 or higher, then you may want to consider the Review Blitz with Practice Exams until you're scoring an 85% per the CCO Proven Process.

A Review Blitz includes approximately two (2) days of material. They are not a substitute for a full course and using them exclusively as a tool for passing the AAPC without a prior full course will result in a failing score. The Review Blitzes are only meant to refine, review and refresh existing knowledge from a full course taken at CCO or somewhere else. It is not a full course nor do we advertise it as such anywhere. It also teaches exam strategy and test-taking tips.

A Full Course includes approximately 40-80 hours of core material meant to teach the material from scratch.
