Resolved 74230 Barium Swallow

What code would be used for Aspiration seen with all liquids during a barium swallow? I know its not a T code but could not find a proper diagnosis code.

Perhaps this code fits.

Y84.4

Aspiration of fluid as the cause of abnormal reaction of the patient, or of later complication, without mention of misadventure at the time of the procedure

What would a primary DX code be? Would you go with the reason for the barium swallow if there is no complications from the aspiration? We been having denials for the use of a asphyxiation T code. Would an asphyxiation code apply for aspiration found during the study? I could not index aspiration and medical necessity requires a secondary code. Would the Y84.4 only apply if the T code was used like an asphyxia? As the Y84.4 could not be billed alone as a primary right?

If you are billing for the diagnostic test then use the reason for the study. Then you use the dx code given by the provider in the clinic.
