Patricia Johnson asked:

Q.21)
A patient with malignant pleural mesothelioma who is not amenable to curative surgery presents for his
second round of chemotherapy treatment at the chemotherapy day unit in the outpatient hospital. Using a
previously established PICC line the patient receives 1000 mg pemetrexed by infusion for 10 minutes. This is
followed by 1000 ml IV fluid for hydration. Then at 30 minutes after the pemetrexed administration; 150 mg
cisplatin is infused for 2 hours. What code(s) are reported?
A. 96413; 96415 X2; J9305 X100; J9060 X30; C45.0
B. 96413; 96415; 96417; 96361; J9305 X100; J9060 X15; Z51.11; C45.0
C. 96413; 96415 X2; 96417; 96361; J9305 X100; J9060 X15; Z51.0; C45.0
D. 96413; 96415; 96417; J9305 X15; J9060 X100; Z51.11; C45.0

Tiffany Baca wrote: The total time of the chemotherapy infusion administration is 2 hours and 10 minutes. Then we have 2 different chemotherapy medications which are pemetrexed and cisplatin. You would report code 96413 for the first chemo administration of pemetrexed which was administered over 10 minutes; which the code description says up to 1 hour. You would report code 96417 for the second chemo administration of cisplantin; which was administered for 2 hours; because cisplantin is a different drug then pemetrexed. If you look at the parenthetical note under 96417 it states "report 96415 for addition hour(s) of sequential infusion" so the sequencing is 96413; 96415 and 96417. Code J9305 is for pemetrexed which is packaged in 10 mg units and the patient received 1000 mg (1000/10 = 100) and would be coded as J9305 x100 . Code J9060 is for cisplatin which is packaged in 10 mg units and the patient received 150 mg (150/10 = 15) and would be coded as J906 x15. Z51.1 is for the
encounter of the chemo and code C45.0 for the malignant pleural mesothelioma.
