Only confirmed cases (the provider's diagnostic statement that the patient is HIV positive or has an HIV- related illness is sufficient) are coded. According to the reason for encounter B20 can be coded as either principal diagnosis or as additional diagnosis except for code D59.31(Infection-associated hemolytic- uremic syndrome) is coded as principal diagnosis.
If a patient without any documentation of symptoms is listed as being HIV positive/known HIV/HIV test positive then code Z21(Asymptomatic human immunodeficiency virus) code is used.
If a patient with inconclusive HIV serology, but no definitive diagnosis or manifestations of the illness assign code R75.
Once a patient has developed an HIV-related illness, the code B20 should be coded on every subsequent encounter.
If a patient documented with HIV disease is currently managed on antiretroviral medications assign codes B20, Z79.899(Other long term drug therapy).
