This will depend what type of risk adjustment work you are doing. If you work with an MA plan you will not have contact with the providers. That being said if you do have the ability to contact the provider you can use error and omission codes. E/O codes and for the most part they mean the same thing across the board.
These include: Not valid signature; No DOS; or Illegible documentation. The company you work for will give you access to the E/O codes they use.
